Intramuscular lipoma is a deep-seated lipoma that arises in the muscle and, due to its infiltrative nature mimicking malignant tumors, it poses a great concern for clinicians and pathologists. Presentation of an intramuscular lipoma in the paraspinal muscles is extremely rare. In this report, we presented a rare case of thoracolumbar paraspinal infiltrative lipoma complicated with paravertebral abscess in a chronic renal failure patient.
